Tại sao nên khám tiền hôn nhân?

Hôm nay mình giới thiệu sơ lược về nguyên nhân tại sao các bạn nên khám tiền hôn nhân. 

Sức khỏe tổng quát

Mục đích của việc khám tiền hôn nhân là để giúp các cặp đôi nắm rõ về tình trạng sức khỏe tổng quát của bản thân nhất là sức khỏe sinh sản. Việc khám tiền hôn nhân hiếm khi trả lời chắc chắn rằng các bạn có thể có con được hay không, điều mà các bạn hay hỏi mình khi tư vấn. 

Sức khỏe sinh sản

Việc xác định hai bạn có thể chắc chắn có con hay không sau khi bác sĩ trả lời rằng tình trạng sức khỏe sinh sản của các bạn hoàn toàn bình thường còn phụ thuộc vào quá nhiều yếu tố trong đó bao gồm nhiều nguyên nhân cực kỳ phức tạp từ cả hai phía trừ những nguyên nhân quá rõ ràng về việc không thể có con như người nam không hề sản xuất được tinh trùng và người nữ không có tử cung. 

Bệnh di truyền

Bên cạnh đó việc khám sức khỏe tiền hôn nhân cũng để nhằm sàng lọc những căn bệnh di truyền mà chẳng may hai bạn đều mắc phải, điển hình như bệnh Thalassemia.

Lư ý:

Việc khám tiền hôn nhân không quyết định việc các bạn có nên kết hôn hay không, việc các bạn kết hôn là vì hạnh phúc của các bạn mang đến cho nhau, không phải từ thông báo tình trạng y khoa của bác sĩ.

Bác sĩ sẽ giải thích cho bạn rõ tình trạng hiện tại và những tiên lượng xấu về tương lai sinh sản của các bạn mà thôi. 

Hôm nay mình sẽ tập trung phân tích vì sao các bạn nên sàng lọc căn bệnh Thalassemia này nhằm cung cấp cho các bạn một số hiểu biết nhất định về hậu quả của nó mang lại về mọi mặt như tâm lý, kinh tế, chất lượng dân số trong tương lai. 

BỆNH THALASSENMIA

Ở Việt Nam, mỗi năm có trên 8000 trẻ sinh ra bị bệnh Thalassemia trong đó có trên 2000 trẻ bị bệnh nặng và trên 800 trường hợp bị phù thai phải chấm dứt thai kỳ. 

Mức độ nguy hiểm

Nếu trẻ sinh ra không may mắc bệnh Thalassemia có triệu chứng cần điều trị thì việc điều trị hầu như phải kéo dài cả đời, chi phí điều trị vượt quá khả năng của cá nhân và nguồn lực quốc gia bao gồm chi phí truyền máu và thải sắt, thậm chí cắt lách, chi phí nằm viện, di chuyển đến trung tâm có điều kiện để điều trị, đối với những trung tâm tiên tiến còn tốn chi phí cho việc ghép tế bào gốc, ghép gene.

Ngoài ra trẻ bị bệnh Thalassemia còn bị ảnh hưởng đến nhiều cơ quan trong cơ thể như: gan, tim, nội tiết, hệ xương… có thể gây chậm phát triển về thể chất. 

Việc tầm soát

Việc tầm soát căn bệnh này khá đơn giản, cặp đôi sắp kết hôn chỉ cần làm xét nghiệm gọi là CÔNG THỨC MÁU, xét nghiệm này sẽ trả lời cho các bạn biết mình có khả năng mắc Thalassemia hay không, nếu kết quả trả về bạn có hồng cầu nhỏ nhược sắc (MCV < 80 fl, MCH < 26 pg) thì bác sĩ có thể  làm thêm một số xét nghiệm để chẩn đoán xác định như sắt huyết thanh, ferritine, transferine, LDH, điện di hemoglobin thậm chí sinh học phân tử.

Cả hai bạn chỉ nên làm chẩn đoán xác định Thalassemia khi cả hai đều có hồng cầu nhỏ nhược sắc, nếu một trong hai bạn có kết quả công thức máu hoàn toàn bình thường thì không cần xét nghiệm chẩn đoán Thalassemia.

Bệnh Thalassenmia có di truyền?

Nếu một trong hai bạn nam hoặc nữ bị Thalassemia thể ẩn thì khả năng cho thai: 50% khỏe mạnh, 50% Thalassemia thể ẩn giống bố hoặc mẹ nghĩa là chỉ biệu hiện thể tích hồng cầu nhỏ nhược sắc có kèm thiếu máu hoặc không và thường ít khi cần điều trị. 

  • Nếu cả bạn nam và nữ đều bị Thalassemia thể ẩn thì theo quy luật di truyền trong một lần thụ thai sẽ có các khả năng cho thai như sau: 50% Thalassemia thể ẩn, 25% khỏe mạnh và 25% bị bệnh Thalassemia.
  • Nếu một trong hai bạn bị Thalassemia (người mang gen đột biến đồng hợp tử ) thì khả năng cho thai trong một lần thụ thai: 50% bị bệnh Thalassemia, 50% Thalassemia thể ẩn.

 Từ kết quả đó các bạn sẽ được tư vấn di truyền thích hợp. 

Thực tế hay gặp là do các cặp đôi ít khi đi khám kiểm tra trước khi kết hôn và sau khi kết hôn với nhiều lần mang thai thất bại mới biết cả hai đều mang gen đột biến Thalassemia, điều này mang lại nỗi đau về mặt tâm lý rất lớn khi người phụ nữ buộc phải chấm dứt thai kỳ hoặc thai chết trong tử cung giai đoạn sớm (thai lưu). 

Muốn có con khi bị bệnh Thalassemia thì làm sao?

một cặp vợ chồng sau khi được xét nghiệm di truyền tìm đột biến Thalassemia và có kết quả cả hai người đều mang gen đột biến dị hợp tử Thalassemia thì liệu có giải pháp nào tốt hơn cho cả hai nhằm tránh nguy cơ phải chấm dứt thai kỳ nếu để mang thai tự nhiên sau đó mới chọc sinh thiết gai nhau, chọc ối hay lấy máu cuống rốn của thai để phân tích di truyền. 

Xét nghiệm đột biến di truyền đơn gen giai đoạn tiền làm tổ (Preimplantation Genetic Testting for Monogenic disorder- PGT-M) nghĩa là dù hai vợ chồng không bị hiếm muộn nhưng vẫn phải làm thụ tinh trong ống nghiệm để sinh thiết phôi ngày 5 và phân tích di truyền sau đó trữ đông phôi để chuyển phôi vào tử cung sau khi có kết quả di truyền của phôi khỏe mạnh bình thường tức phôi không mang đột biến đồng hợp lặn. 

Việc sinh thiết phôi mang lại hy vọng điều trị cho những chị em trước đó đã mắc bệnh dù còn nhiều tranh cãi về vấn đề đạo đức nhưng cũng có khả năng không có phôi chuyển vào tử cung vì sau khi có kết quả di truyền có thể tất cả các phôi đều mang đột biến đồng hợp lặn, điều này làm tăng chi phí điều trị đối với cặp vợ chồng vốn đã chọn thụ tinh trong ống nghiệm nhiều tốn kém.
